We are proud to announce we have received 3 years National Lottery Funding for a Brain Injury Community Social Prescribing Link Worker.

The role will empower people in the community and help them take control of their health and well-being. The Brain Injury Community Social Prescribing Link Worker will spend time with people to focus on, ‘what matters to me’. They will take a holistic approach, connecting people to community groups and statutory services for practical and emotional support.

Social prescribing can help to strengthen community and personal resilience and reduces health inequalities. The role will help address the wider determinants of health, such as physical inactivity. The Link Worker will increase people’s active involvement, support people with their rehabilitation, including support for mental health, and those who are lonely or isolated, thus increasing an individual’s health and well-being. 


Who are we looking for:

We are looking for someone who has at least 1 years’ experience dealing with complex health issues, disabilities and mental health in a community setting. You must be confident, with a positive ‘can-do’ attitude and the ability to work to Key Performance Indicators.  We are looking for someone who focuses on what people can do, and not what they can’t and a clear understanding of working in a person-centred manner.  Travel in East Kent is a critical element to this role and you must be able to drive and have access to a vehicle.
